Health Dialog's Pharmacy Solutions team is looking for Certified Pharmacy Technicians (CPhT) to work as Care Navigators in the state of Texas. This position is a remote, work from home opportunity but you must live in Texas.

What does a Care Navigator- CPhT do?

• Perform primarily outbound telephone calls to patients to discuss medication adherence (taking the medications as prescribed by the provider, following medication directions)

• Use scripted communication prompts to reach, engage, and provide medication and condition education, complementary resources, and reconcile medication related activities

• Answer patient questions, concerns, and address identified barriers to medication adherence

• Complete services on patient's behalf, including refill and 90-day supply conversion requests

• Does not include any insurance adjudication or related activities

What skills and experience are required?

• High School Diploma or General Education Degree (GED)

• Actively Registered in good standing as a Pharmacy Technician in Texas

• Active National Pharmacy Technician Certification (eg PTCB, NHA)

• 2 years? experience working as a Pharmacy Technician in a healthcare or retail pharmacy setting; experience in call center or phone support role preferred

• Excellent verbal and written skills in English; fluent in Spanish a plus

• Experience with common medical pharmacy terms and medications names

• General knowledge of therapeutic categories and common indications of medications; ability to use pharmaceutical references.

• Strong computer skills?Microsoft applications (such as Word and Excel) and ability follow written and verbal instructions to assembly computer set-up at home

• Display a high degree of dependability in personal attendance and punctuality.

Shift: Shifts will vary between the hours of 10:00 AM?9:00 PM ET, Monday through Friday. Full-time position.

Location: This position will be remote working from home but you must live in Texas. Candidate must be able to meet Health Dialog's requirements for working from home.
