GENERAL SUMMARY AND PURPOSE:

Provides pharmaceutical care to customers of Saint Alphonsus Regional Medical Center.

Antimicrobial Stewardship

Required 1ualifications: Keep the usual pharmacist required qualifications - PharmD, licensure requirements - do we want to say an ASHP accredited PGY1 residency or equivalent clinical practice as required (2-3 years of institutional experience?) I want to say yes, maybe we do two years since it would be listed as required just to keep options open?

Preferred Qualifications: Successful completion of an ASHP accredited specialty residency (Infectious Diseases Strongly Preferred). Some job descriptions also list Critical Care as an option.

Example Job Duties: this position would support all four hospitals and outpatient stewardship efforts

Perform prospective and retrospective antimicrobial review, feedback and intervention throughout the Saint Alphonsus Health System (inclusive of SAMG).

Participate in the development of and updating of infectious disease guidelines and decision support for clinicians relating to the selection, dose, duration and monitoring of antimicrobials

Represent antibiotic stewardship to front-line prescribers and administration leaders

Actively lead related committees, groups or initiatives relating to antimicrobial stewardship

Submit and analyze antibiotic utilization data utilizing internal and external databases (ex. CDC NHSN AU Module)

Antibiogram development

Review requests for restricted antibiotic use

Assist in the development of annual quality, safety and financial goals for anti-infective utilization

Participate in and conduct didactic and experiential training of pharmacists and pharmacists in training on the principles of antimicrobial stewardship and infectious diseases (they will precept residents and potentially students on an infectious diseases rotation)

Coordinates continuing education activities for provider, nursing and pharmacy staff; orients new colleagues on antimicrobial practices and procedures; coordinates/contributes submission to pharmacy newsletters, journal clubs and in-services to professional staff

Represent SAHS in Trinity Health pharmacy initiatives regarding antimicrobial stewardship

SKILLS, KNOWLEDGE, EDUCATION AND EXPERIENCE:

Doctor of Pharmacy or Bachelor of Science in Pharmacy required. Residency preferred. Must be licensed as a Pharmacist with the Idaho and Oregon State Boards of Pharmacy. Six months of experience preferred.

ESSENTIAL FUNCTIONS:

Knows, understands, incorporates, and demonstrates the Organization's Mission, Vision, and Values in behaviors, practices, and decisions. Demonstrates knowledge and skills to competently care for all assigned age groups (Neonate, Child, Adolescent, Adult, Geriatric as applicable). Revenue management: ensures the accuracy of documenting services and supplies provided to the patients. Reviews patient history incorporating various pharmacy activities providing customers with adequate education for optimum outcomes. Prepares pharmaceutical products for patients' use by safe and cost effective methods using acceptable professional standards. Comprehends and supports quality assurance and risk management issues.

Participates in various departmental duties; material resource management, clerical and financial responsibilities, safety, and other issues. Supervises technicians and pharmacy support staff. Excellent communication skills Adaptable to institutional roles and schedules.
