JOB SUMMARY/PURPOSE Ensures departmental efficiencies and compliance with regulatory bodies. Provides clinical pharmacy services to ambulatory patients. Supervises and instructs other pharmacists and pharmacy students. DEPARTMENT DESCRIPTION The Anticoagulation Clinic provides comprehensive medication management of oral and subcutaneous anticoagulation. Services include medication monitoring and perioperative risk assessment and bridging therapy. The ambulatory care pharmacists in this department also provide anemia management with erythropoesis stimulating agents and general medication therapy management services. EXPERIENCE/EDUCATION/QUALIFICATIONS Current unencumbered Oregon Pharmacist Licensure required. Healthcare Provider Level BLS required within three (3) months of hire. Pharmacist Immunization Certification preferred. Previous hospital pharmacy experience preferred. Experience working in a clinic based anticoagulation service preferred. Alternatives may include: Completion of a residency based anticoagulation rotation, formal training in anticoagulation services, or certification from an approved course in anticoagulation therapy management. Ability to perform group presentations and listen to and understand complex information and ideas.# Computer Literacy - Knowledge of electronic equipment, computer hardware and software, including applications and programming.
