Parkview Regional Medical Center (PRMC) is accepting applications for a clinical pharmacy specialist in ambulatory care.## Position Details:# The clinical pharmacy specialist will provide direct patient care under collaborative practice agreements as part of an interdisciplinary team. Ambulatory pharmacists throughout Parkview provide care in hospital-based outpatient clinics and as an embedded resource in physician-based clinics throughout Parkview Physician Group. This position, based at the PPG Ohio Montpelier Family Medicine clinic, includes clinic-based anticoagulation management, chronic disease management via a collaborative practice agreement, and Medicare Annual Wellness Visits. The pharmacist will work collaboratively with physicians, nursing, medical assistants, and support staff to provide pharmaceutical care to ensure safe, appropriate, and cost-effective therapies. This includes medication reconciliation, education, facilitation of pharmacy benefit utilization to optimize adherence, and optimizing transitions of care. This position will facilitate the expansion and enhancement of clinical pharmacy services across the ambulatory environment. Pharmacy licensure or eligibility in Indiana is required.
