The UW MEDICAL CENTER - MONTLAKE PHARMACY has an outstanding opportunity for a PART-TIME AMBULATORY CLINICAL PHARMACIST - SPECIALTY PHARMACY PROGRAM (50%, Day Shifts). The Ambulatory Clinical Pharmacist (Pharmacist 3) will provide clinical pharmacy services to specialty clinic patients. This position will work with the health-care team to implement and monitor specialty medication therapies for the achievement of the desired therapeutic outcomes. Additionally, the Ambulatory Clinical Pharmacist will provide drug therapy consults and participate in treatment protocol development, provide specialty pharmacy and indigent care program information and education to providers, patients and medical staff, and precept pharmacy students and residents and serve as a drug information resource for health care team to ensure patients have access to needed medications and provide clinical support and documentation when required.

RESPONSIBILITIES:

Clinical Management:

* Identify patient needs and ensure therapy management is complete, including but not limited to lab monitoring, side effect management, compliance issues/concerns, therapy response.
* Meet with patients in clinic or counsel via telemedicine or over the phone as necessary to assess therapy response, ensure patients understand their medication therapy, address side effects and assist with compliance issues.
* Ensure patients' specialty prescriptions are received by the patient.
* Function as the pharmacy liaison for patients receiving prescriptions at a UW Medicine Specialty Pharmacy.
* Help develop a plan to improve, streamline, and increase the efficiency and effectiveness of the specialty pharmacy program. Work with the Specialty Pharmacy team to evaluate and further develop systems and processes including: Developing a standard case management plan for disease states of the patients supported by UW Medicine Specialty Pharmacy. Contribute to the optimization of software for case management documentation, tracking, and patient communication.
* Help develop and implement clinical protocols to optimize safety for specialty medications with Risk Evaluation and Mitigation Strategies (REMS) requirements.
* Function as patient advocate to justify prescribed therapy or recommend alternative therapies when needed for insurance approval.
* Serve as a drug information resource for health-care professionals, patients and their families.
* Document in the patient's electronic health record or other appropriate system, patient care activities including but not limited to: pharmacokinetic consultations, patient education notes, medication histories, specialty pharmacy re-assessments, pharmacotherapeutic interventions, adverse drug reactions and other significant recommendations or activities.

Education:

* Provide education to patients about their specialty medications.
* Serve as a subject matter expert for patients physicians, administrators, clinical staff, pharmacy staff, and third party payers for clinical issues regarding specialty pharmacy.
* Precept pharmacy students and residents.
* Participate in collaborative research projects with healthcare professionals, pharmacy residents and student pharmacists.
* Assist in providing education of Specialty Pharmacy to pharmacy, clinics, social workers, and physicians via presentations, newsletters, emails, and website.
* Attend seminars and stay updated on specialty pharmacy therapies and insurance issues.

Other:

* Perform the duties of a Pharmacist 2 and other related duties as required.
* Follow UW Medicine security and confidentiality policies and procedures.
* Maintain current professional standards and credentials through study materials and education to stay abreast with the continuous changes in the health care industry. REQUIREMENTS: Two years of experience in pharmacy practice. Months spent in the completion of an American Society of Health System Pharmacists accredited residency training program may substitute month-for-month for the experience.

LEGAL REQUIREMENT: State of Washington Pharmacist License.
