Job Summary:

The Virtua Integrated Network (VIN) Ambulatory Clinical Pharmacist is responsible for the provision of safe, effective, appropriate and cost-efficient medication management in assigned primary care practices in collaboration with healthcare providers and patients.

Position Responsibilities:

Monitors patients in a specific unit/specialty service, including collecting patient-specific data, interpreting drug-related laboratory data, performing relevant physical assessments, interviewing patients and identifying patient goals of therapy.

Assesses patient-specific medical problems, prioritizes medical problems based on urgency and severity, and identifies preventive and health maintenance issues.

Evaluates patient-specific drug therapy by evaluating appropriateness of drug therapy, assessing effectiveness, and identifying potential interactions or adverse effects. Reports and documents drug errors, adverse drug reactions, allergies and patient adherence issues.

Designs a comprehensive drug therapy and patient education plan for patient-specific problems and persuasively justifies recommendations based on patient-specific pharmacologic, pharmacokinetic, pharmacodynamic, pharmacogenomic, pharmacoeconomic, ethical, legal and evidence-based information.

Relays drug therapy recommendations to clinicians and documents clinical interventions in the electronic health record in a timely, professional and effective manner.

Collaborates with patients, caregivers and other healthcare professionals.

Prepares drug monographs and cost analysis for presentation to committees, physicians and other staff as requested. Possesses and maintains knowledge of current evidence-based practice for applicable chronic disease states and serves as a drug information resource for other healthcare providers and team members.

Represents VIN Clinical Pharmacy Services on various committees, staff meetings, and professional meetings as needed as it relates to the pharmacy area of practice/specialty.

Coordinates continuing education activities for medical and nursing staff; orients new employees on pharmacy practices and procedures. Includes submissions to the pharmacy newsletters, participating in journal clubs, and providing in-services to professional staff.

Serves as a preceptor to pharmacy residents, students, and volunteers.

Position Qualifications Required:

Required Experience:

Completion of an American Society of Health-System Pharmacy accredited Postgraduate Year One (PGY1) Residency (required). Postgraduate Year One (PGY1) Residency or Postgraduate Year Two (PGY2) Residency in Ambulatory Care (preferred). Three to five years of recent relevant pharmacy experience preferred .

Required Education:

Doctor of Pharmacy degree or Bachelor of Science in Pharmacy with commensurate experience.

Training / Certification / Licensures:

Possession of a valid registered pharmacist license to practice in New Jersey (required).

Board certification (BCPS, BCACP or CGP) is preferred.

Relevant experience - Ambulatory Care Pharmacy Residency OR extensive experience with patient counseling (inpatient or outpatient) and clinician interaction OR experience as a pharmacist in primary care practices OR experience in Medication Therapy Management clinic
