SUMMARY:

Supervise technical personnel, interns, and students in assuring completion of departmental tasks. Responsible for assuring that dispensing and pharmaceutical care services are carried out in accordance with all federal, state and local laws .

ESSENTIAL JOB FUNCTIONS:

* Responsible for assuring that dispensing and pharmaceutical care services are carried out in accordance with all federal, state and local laws.
* Reviews direct oral or written physician's order against patient medication profile for appropriateness of drug, dosage, regimen, and for medication allergies, duplication of therapy, and drug/drug interaction; contacts the physicians, prescriber or nurse to resolve or clarify any order in question.
* Researches and answers drug information questions, including determination of appropriate drug, dose, and/or calculations, route, schedule, and appropriate use with regard to drug compatibility, drug interactions, biopharmaceuticals and pharmacokinetics.
* Performs all clinical (pharmaceutical care) functions thoroughly and accurately in a timely manner.
* Maintains patient profile through computer entries for patient data entry of new orders, and order maintenance.
* Compounds and dispenses pharmaceutically elegant dosage forms as ordered and assures delivery of the medication to the correct patient in a timely manner. Checks all new and routine medication orders compounded or filled by pharmacy technicians and interns and checks technical order entry through order confirmation in accordance with all federal, state and local laws. Accurately fills, labels, and dispenses outpatient prescriptions and effectively counsels' patients on the correct use of their medication(s).
* Maintains the formulary system of drug selection and use; notifies prescribers of therapeutic alternatives when medication order is non-formulary or needs further clarification.
* Participates in patient education including patient counseling of drug therapy and formal patient education programs.
* Participates in staff development and educational programs for personnel. This includes in-service lectures to pharmacy, medical and nursing staffs, and pharmaceutical publications.
* Aids in the instruction (during the on-the-job phase) of new employee orientation and training.
* Assists in maintaining inventory control of all pharmaceuticals and controlled substances (and associated control records) as per department procedures.
* As part of daily operations and/or special assignment, participates in all aspects of the department quality improvement program.

EDUCATION/QUALIFICATIONS:

Required Minimum Education: 4 year/ Bachelors Degree

Specialty/Major: Pharmacy

Preferred Education: Post Graduate Degree (Ph.D)

Specialty/Major: PharmD

Licensure/Certification Required: Licensed Pharmacist in the State of Ohio.

Licensure/Certification Preferred: Board Certified in Pharmacotherapy.

Minimum Years and Type of Experience:

* Must have attained experience in the various aspects of pharmacy practice either as a licensed pharmacist or through an accredited internship or externship program.

Other Knowledge, Skills and Abilities:

* Required: Working knowledge of the practice of pharmacy, including dispensing activities and pharmaceutical care activities that encompass knowledge of appropriate drug use, drug interactions and incompatibilities, biopharmaceuticals, and pharmacokinetics.
* Thorough knowledge of the federal, state and local laws as they pertain to the practice of pharmacy.
* Must be exceptionally well versed in communication skills and capable of working effectively with department personnel, medical staff, nursing staff, and other hospital personnel.
