Job Summary

Performs the activities within the GHS Referral Center to grow the referral volume from outside health care facilities and physician offices.  Accomplishes the responsibilities of this position by integrating and maximizing the work of various disciplines, resources and services across departmental lines.

 Maintains Quality and Compliance for the GHS Referral Center.

 Works closely with internal and external referral sources to assure a smooth transition and to assure compliance with applicable state and federal regulations

 Collaborates as appropriate with other departments to ensure efficient processes and facilitate problem solving. 

 Uses advanced level nursing skills to assess and prioritize different types of transfers

 Operates within the established budget set forth for the GHS Referral Center. Coordinates patient admissions and transfers and bed availability on specified clinical areas.  Evaluates admission/transfer process for patients, families, and physicians on an individual and collective basis.

Supervisory/Management Responsibilities

This is a non-management job that will report to a supervisor, manager, director or executive.

Minimum Education

Associate's Degree - Nursing

Minimum Experience

3 years - Acute Care Nursing

Required Certifications/Registrations/Licenses

RN - REGISTERED NURSE - NLRN

1 year of critical care, referral center, and/or case management experience - Preferred

BLS - Required

Customer Service Skills - Preferred
